The basic premise of Toilet-bound Hanako-kun is set at Kamome Academy, a school where seven famous "School Mysteries" and other supernatural rumors circulate. The story follows Nene Yashiro, a romantic middle school student who loves the occult, who, desperate for love, decides to summon the seventh and most famous mystery: the "Toilet-bound Hanako-kun," said to grant wishes to whoever summons him in the third stall of the girls' bathroom on the third floor. To her surprise, Hanako-kun is not a girl, but the ghost of a young boy who resides in the toilet. After a series of events, Nene becomes bound to Hanako-kun as his human assistant, and along with Kou Minamoto, a young exorcist, they begin to resolve supernatural incidents within the school, maintain the balance between the world of the living and spirits, and uncover the mysteries behind Hanako-kun's past.

Hanako-kun and Nene are accompanied by various key characters essential to the plot and contributing to the series' dynamic. Hanako-kun is the Seventh School Mystery, a playful, mischievous, and sometimes dark ghost who acts as the guardian of balance between worlds.
