The basic premise of Dorohedoro is set in a dystopian, violent, and bizarre world divided into two main regions: The Hole, a filthy and dangerous city where ordinary people live and serves as a testing ground for sorcerers, and the Sorcerer's World, where sorcerers live with their magical abilities. The story follows Caiman, a man with a reptilian head and amnesia, who seeks his identity and the sorcerer who transformed him. He is accompanied by Nikaido, his dynamic friend and restaurant owner. Caiman's peculiar method of finding information is to bite sorcerers with his head, revealing a mysterious man in his mouth who speaks to him. The plot is full of mysteries, bloody battles, dark humor, and a complex interaction of characters from both worlds.

Caiman is accompanied on his dangerous journey by various key characters who add to the series' complexity and humor. Nikaido is his loyal friend and companion, an exceptionally skilled fighter with a mysterious past, who manages the "Hungry Bug" restaurant. In the Sorcerer's World, two of the most important characters are Shin and Noi, a duo of enforcers who work for their boss, En. Shin is a tough sorcerer with a heart-shaped head, skilled in surgical magic, while Noi is a tall and muscular sorceress with healing abilities. Despite the violent nature of their work, they possess a peculiar sense of morality and a deep friendship. These characters, among others, create a dynamic ensemble that drives the plot forward.

Dorohedoro is characterized by dark, violent, and absurd themes, as well as a unique art style. Central to it is Caiman's search for identity and revenge, but the series also explores the concept of "family" formed by choice in a chaotic world. The eerie and grotesque element is dominant, with depictions of body horror and bloody scenes, but always with a dash of dark humor. Q Hayashida's art style is recognizable for its gritty, linear drawing, detailed backgrounds depicting decay and messiness, and unique character designs. The ambiguity between "good" and "evil" is a recurring theme, as even the seemingly "villainous" characters have their own values and motivations.
